Patient Forms

If you would like to expedite your visit to Dr. Wandzel, you may print and complete patient forms at home. When you arrive at our office, please have your insurance card(s) and the completed forms ready for the receptionist.

Health benefit forms (such as short term disability packets) can be submitted in person to our office during normal business hours. Please allow 7 days for completion. There will be a $15 charge for this service.

Patient Portal

This patient portal will allow you to view your billing and medical information. Before accessing this private content, you will need to obtain a username and password from our office.

Prescriptions

Most prescription refills are called into your pharmacy the same day they are requested. However, in the event that we are busier than usual, please allow up to 2 business days for prescription refills to be completed.
